HIS Upload Accepted vs. Submission Completion
Submitting your Hospice Item Set (HIS) form is critical to your hospice agency’s financial health. CMS will not proactively notify your agency that records have been rejected until they send a bill for up to 4% of your claims revenue.
It is a vital best practice after uploading your HIS zip file click the Submission Status tab and confirm that the submission status is Completed, that CMS accepted the content of the upload, and not just rely on the upload having been accepted.

While creating and exporting HIS admission/discharge records is a super-simple and transparent process that takes place in the Hospice Tools HIS dashboard, uploading the HIS zip file takes place in the CMS QIES system and only the hospice agency has access to the QIES system and visibility on statuses of uploads.
