Easily Capture the Right Caregiver Information for Better Care & CAHPS Survey Response Rates

“Select the right caregiver to receive the survey. Don’t simply default to the spouse or P.O.A. Here is a section from  the CMS  Quality Assurance Guidelines v6.0, Sept. 2019: One caregiver category does not automatically have preference over another. Hospices should not prioritize a primary informal caregiver that is a family member over a friend, as a friend may have more knowledge regarding the decedent’s hospice care than a family member.”

Relationship has been changed from a free-text field to a drop-down box with many common relationships. This can help minimize errors from typos and improve reporting.

 

If a specific relationship option does not apply, you can select “Other (family member)” or “Other (non-family member)” and enter custom information in the Other Note/Info field.

This field can also be used to capture quick notes that will aid in communications with a caregiver.

 

The Legal Status field can be used to capture and additional status that a caregiver has, such as POA or Guardian.
